We provide clinically supervised, fall-prevention and dementia focused home care for high-risk seniors in Etobicoke, North York, and Vaughan. What does our Home Care Services offer?
We specialize in clinical, high-risk senior care — including fall prevention, dementia and behavioral support, and post-hospital recovery.
Our trained Personal Support Workers (PSWs) provide personalized daily support including bathing, mobility, medication reminders, companionship, meal preparation, safety supervision, emptional wellbeing, and all Activities of Daily Living.
Every client receives a customized care plan with professional oversight, required documentation, and backup PSW coverage.
For complex or high-risk needs, our Registered Practical Nurses (RPNs) conduct a comprehensive assessment, create an individualized medical care plan, provide monthly check-ins, and supervise ongoing care.
Services include medication administration, wound care, diabetic management, chronic condition monitoring, dementia behavioral management, palliative care, and 24-hour high-dependency support.
